Skilled Nursing
Skilled home health nursing care is the provision of intermittent skilled services to a member in the home for the purpose of restoring and maintaining his or her maximal level of function and health. These services are rendered in lieu of hospitalization, confinement in an extended care facility, or going outside of the home for the service.
Physical Therapy
Skilled therapy services: Physical, speech, and occupational therapy services that are reasonable and necessary for treating your illness or injury and performed by or under the supervision of a licensed therapist. Physical therapy includes gait training and supervision of and training for exercises to regain movement and strength in a body area.
Occupational Therapy
Occupational therapy helps you regain the ability to do usual daily activities by yourself, such as eating and putting on clothes.
Speech Therapy
Speech-language pathology services include exercises to regain and strengthen speech and language skills.
Home Health Aide
Assists patients by providing personal services, such as, bathing, dressing, and grooming.
Medical Social Worker
Assess Social and Emotional concerns related to the patient’s illness, provide counseling and help find resources in the community.
